The Service Coordinator is an individual on the early intervention team who will help the family identify appropriate services and coordinate the professionals who will work with a child.  Together, the family and the Service Coordinator work to monitor, review and evaluate the family’s Individualized Family Service Plan (IFSP). 

 

Some key outcomes in service coordination is to ensure that each legal guardian/parent is well informed of their rights, opportunities and responsibilities; assist in reviewing and making informed decisions about financing services for a child and family; assist in arranging for various assessments or evaluations for a child; work with families to identify and schedule the IFSP team; help families get linked with other parents or parent support groups, advocacy organizations or other information sources they identify as important to them, and work closely with families to identify planning needs for various transitions that may affect services for their child and family.
